Completion of all steps below confirms your place on a Queen City Pro-Am roster.

STEP 1: complete the player questionnaire

Before you're eligible to participate, we need to learn a bit more about you.

STEP 2: SUBMIT YOUR REGISTRATION FEE

You’ve made the cut, but your spot isn’t locked until payment is in. Registration Fee must be completed by June 21st — miss the deadline, and your spot goes to a hungry free agent ready to suit up.

STEP 3: WAIVER FORM (COMPLETED IN PERSON)

All players must sign a liability waiver before their first game. This will be available at check-in on Opening Game Day.​
​NO WAIVER, NO PLAY.  Please arrive early on your first game day to complete and sign the waiver on-site.
